Wellness and Prevention Initiative

Healthy For Life is a program that focuses on the prevention of childhood obesity. It was funded in part by a grant of the Wellness and Prevention Initiative.
The SJHS Foundation seeks to promote the health and well-being of low-income individuals and families in communities surrounding our hospitals through the Wellness and Prevention Initiative. By funding proactive programs through the grant cycles of the Wellness and Prevention Initiative, the SJHS Foundation seeks to eliminate the need for catastrophic care and its associated social, emotional and economic costs.
The title of the initiative describes its central vision and strategic focus. Wellness programs address the health and well-being of the individual or community at risk or with pre-existing health condition. Prevention (i.e., primary, secondary, tertiary) programs are part of a comprehensive strategy to improve the health status and quality of life in our local communities while reducing demand for high-cost medical services. Partnering with other organizations to address the health and well-being of low-income communities is at the core of the initiative. As in previous grant cycles, SJHS Foundation seeks proposals that address the health and well-being of low-income communities in all areas of health including Body, Mind and Spirit.
Wellness and Prevention Grants are available in St. Joseph Health System California hospital areas. The foundation provides funding for community based non profit organizations and SJHS hospital affiliated programs. Funding is granted to initiatives that proactively address strategies to improve health and quality of life for those who cannot afford to pay for health-related services.
Proposals are reviewed for the following emphasis:
- Wellness and/or prevention
- Capacity of persons and groups to become self-sufficient
- Opportunities for innovative health-related education
- Collaboration and partnership with existing community initiatives
- Tangible, measurable outcomes
